REQUIRED FORMS
To apply, you must return one original signed copy of each of the below listed forms by the timeframe determined by the Marion Military Institute. Make sure all entries are clear and legible (printed or typed as appropriate according to instructions on the forms).
a. Cadet Command Form 139-R (Cadet Enrollment Record). Due to the combination of personal data and ROTC related data being requested on the forms, it is advisable that you complete the application with the direct assistance of detachment personnel. Legibility is important. All signatures must be in ink. Instructions for completing this form can be found at Appendix A.
b. An official copy of your high school transcript through the twelfth grade with the cumulative academic GPA annotated thereon.
c. Copy of your SAT or ACT scores, if it is not indicated on your official transcript.
