Required Medical Forms
In addition to completing the "SportsWare OnLine" Medical Records, you MUST complete the following forms:

Physical Examination (completed after June 1)
A copy of your Medical Insurance Card or information MUST be provided to the Cadet Health & Athletic Training Staff during student registration/in-processing or you can fax a copy of your card(s) directly to Cadet Health & Athletic Training at 334-683-2381.
